Need some help with form submission

Question

I am tryingto create a form that will allow user to submit text information and attach photos as well. But I am running into two problems:

1. I have tested simple text submissions, but both notifications emails fail to send. The error message I receive is:

2. How can I add the button that allows user to submit a photo? I have googled the HTML code for it and tried inserting a custom HTML block, but I don't even know if it worked, as the photo doesn't show up in the data table when I look there to check the submission. Am I even looking in the right place?

Any help would be appreciated.

1. I suggest if you review the "Troubleshooting notification email" article at http://howto.caspio.com/automatic-email ... mails.html and share it with your IT (mail server admin) so they can perhaps enable receiving emails from Caspio on your side.

2. To enable photo upload, you can add a "File" field in the table and then include this field in your DataPage and select form element as "File".
