Formatting a results page

I have mock data of people including demographies and used date of births they're all stored in one table. 
 

I created a search and report database however I want there to be only one result for the individuals demographies and the rest of the results be used date of births by individual in rows how would I achieve this?

I have attached an example of what I'm wanting to achieve and for reference the first circled section would be the demographies I want to show and the second circled section would be the additional date of births used.

Hi @MariaT - Based on the sample on your question, I think for you to achieve this, you need to create a details page and report DataPage that is filtered then compile them on a single page for you to create a similar result on your sample. In addition, if you want the result to only return one record, then you can go to "Results Page Options" -> in advance -> use 1 on Total records returned. Refer screenshot:
