Which means you cannot allow users to dynamically add more name and score fields while filling out the form. I will recommend you to create the table based on the biggest possible player number a team can have. And users can simply leave some fields blank if their team doesn't have so many players. I can recommend to use rules to hide some fields and use a virtual radio button to display them should they need to add more. The screenshot below is an example.
