How Do I Create A Test Or Quiz

Is there any way to create quizzes in Caspio?  I saw an answer but I need more information as to Exactly how I need to write multiple choice questions and choose the answer.   

 

Any help will be appreciated.

Questions must be field values in the table and then you  need to select them in your submission page and select Radio button for answers and type in the options

 

Table: Q1, Q2, Q3 ..., User_ID, Date_Submitted
