A form is submitted

I look at it and either approve it or not

If not approve, the user will go back to the form and update some information

I go back in, look at it and approve it or may be not.

 

I want to keep a record of the "reviewer(s)" of the form and each time a form is reviewed I want the comment timestamp and kept.

 

So my comments field may look like this:

 

5/24/2015 12:56 user1: The form was approved, all uploads were check and found correct.

4/30/2015 6:59 user1: The form was incomplete, email was sent to user

03/15/2015 19:01 user2: The form was incomplete, 3 uploads were incorrect, emailed the user

 

Does anyone has any idea on how to do that?

I would really appreciate it.
