How to close or disable form after submission?

I have built a quiz system... which I do not want users to be able to submit a given quiz more than three times. Unfortunately, users have figured out they can just click BACK and submit the form as often as they wish.

Is there a way to disable the submit button after submission, or to close the form after submission?

You can try to have them sign up first to take the quiz and make the combination of name and last name unique or email address unique. Or have a unique field in your table and in the submission page select Auto value as form element and User IP Address as AutoValue type.
