How to create multiple cascading dropdown

Newbie here, How do I create two cascading drop downs, that work with each other. EG, users select their "Name" and it cascades to districts , then from the district cascade I need users to be able to choose cities(cascading drop down) from the district response? How is this done?

You can select dropdown form element for Name. Cascading dropdown for District and use Name as Parent field and Filter by name as well. Again for City select cascading dropdown, Parent would be District and Filter by District.
